blow back because the blows that james recived in all the game, the death of his loved woman,the death of Maria (she looks like mary, and *spoiler* samael used her for remember the sin on james...that is the reason why she dies many times.
james's powerlessness to do anything about it, while he committed the sin of lying to his wife and then kill her when she was sick in the past, but james deceived himself saying that she died of sickness, so he does not remember his own sin,this is the reason why pyramid head kill maria many times to remeber jame's sins)

I feel very inspired with the awensome (and sad song) of Mary Elizabeth Mcglynn "blow back", for the lyrics that I can indentify with james (i guess ):
